Community Health Needs Assessments and Community Service Plans
The Community Health Needs Assessments and Service Plans are reports produced by the hospitals and local county health departments to identify and address community health priorities. These priorities come from the Prevention Agenda, a vision for New York to become the nation's healthiest state by addressing five health priorities: Prevent Chronic Disease, Promote a Healthy and Safe Environment, Promote Healthy Women, Infants, and Children, Promote Mental Health and Prevent Substance Abuse, and Prevent HIV, Sexually-Transmitted Diseases, Vaccine-Preventable Diseases and Healthcare-Associated Infections. The Long Island Health Collaborative served as the convening agent for these organizations to work together to create these documents. The assessments and plans for each hospital and county health department on Long Island can be found on this website and the website of that organization.
