Please read these Terms and Conditions ("T&C") carefully. By using www.LIHealthCollab.org ("Website") or signing up for an account in the Are You Ready, Feet?™ portal ("Portal"), you are agreeing to these terms. This is a legal agreement. The T&C are set forth on behalf of the Long Island Health Collaborative, and the staff of the Long Island Population Health Improvement Program, ("LIHC and LIPHIP"). Any other associated parties, including but not limited to the Nassau-Suffolk Hospital Council, the Healthcare Association of New York State, the Suburban Hospital Alliance of New York State, and the New York State Department of Health, assume no connection to and/or responsibility for this Website and its contents. The T&C apply to information collected via both the desktop and mobile versions of the Website.
Eligibility
As a user of the Are You Ready, Feet?™ physical activity Portal, you must:
- provide your name and email address
- agree to the Terms & Conditions
We encourage all members of the LIHC and LIPHIP, as well as their friends, family, coworkers, and other associates, to engage with the Portal. However, current and former LIPHIP staff and family/household members are not eligible to receive prizes from the randomized prize drawing.
Privacy
Please see "Website Privacy Policy."
Deleting / Deactivating
A user may choose to delete or deactivate their account. By deleting an account, all user information would be erased. Should a user ever decide to create an account again with the same email address, they will be provided a new experience from scratch. By deactivating an account, a user will allow their information to be stored for a return future visit, in which the user may choose to create an account with the same log in credentials.
LIHC and LIPHIP maintain the right to delete or deactivate a user's account at their sole discretion.

Updates
LIHC and LIPHIP maintain the right to update the T&C and any portion of this website without warning or notice.
Giveaways and Prizes
LIHC and LIPHIP host regular giveaways for users of the Portal. There is no entry fee and no purchase necessary to enter giveaways. To be eligible for giveaways, participants must be users of the Portal, and residents of either Nassau or Suffolk county, New York. LIPHIP staff and family/household members are not eligible to enter or win any giveaway. All other employees of the NSHC not funded through the PHIP are encouraged to participate. Users agree not to create multiple accounts in order to obtain multiple entries into the prize drawing.
The potential winner(s) for each giveaway will be selected in a random prize drawing, conducted by LIHC and LIPHIP, from among all eligible entries received for each Entry Period. The potential winner(s) will be contacted by telephone and/or email and/or a traceable method of mail within approximately two (2) business days after the drawing, and they will be required to complete and return all required Prize Claim Documents within seven (7) business days, or said potential winner may be disqualified, forfeit the prize, and an alternate winner may be selected. The winner agrees to the use of his/her first name and initial of last name in any publicity material.
Prizes are chosen by the LIHC and LIPHIP and are subject to their sole discretion. The LIHC and LIPHIP do not endorse these prizes or their manufacturers, and are not responsible for any issues resulting in use of these prizes. The prize is non-exchangeable, non-transferable, and is not redeemable for cash or other prizes. We reserve the right to substitute any prize with another of equivalent value without giving notice. We reserve the right to cancel or amend the giveaways without notice.

Branding Guidelines
The Long Island Health Collaborative logo, the Are You Ready, Feet?™ physical activity Portal and walkability campaign logo, the names Long Island Health Collaborative and Are You Ready, Feet?™ are brand property belonging to the LIHC and LIPHIP and are not to be used or duplicated without the permission of the LIHC and LIPHIP.
