Who should attend these meetings?

The purpose of this work group is to foster collaboration between Long Island hospitals and health departments to meet their state and federal Community Health Needs Assessment (CHNA) and Community Health Assessment (CHA) requirements, respectively. All hospital and health department representatives with an interest or role in their organization's CHNA/CHA process are welcome to attend.

This group is also open to community-based organizations with an interest in participating in this work group to inform hospitals' Community Health Needs Assessments (CHNAs) / Community Service Plans (CSPs) and health departments' Community Health Assessments (CHAs) / Community Health Improvement Plans (CSPs).
